immunizations

A Georgia health form is needed certifying that the child is in good health and has received the required immunizations. These forms are available through the Health Department or your private pediatrician. The form is #3227, 3032 or 3231. According to Georgia guidelines for certificates of immunization (Policy Guide 3231INS) a valid Georgia certificate of immunization must be kept on file by the school and be available for inspection by health officials.

A certificate becomes invalid on the expiration date indicated. If a current certificate has not been submitted within 30 days after the expiration date, the child must be excluded from attendance until a current certificate is obtained. Any school / facility official who does not enforce the requirements and any parent/guardian who intentionally does not comply with the requirements shall be guilty of a misdemeanor. See Official Code of Georgia Annotated, 20-2-771 (h).

The Official Code of Georgia provides for only two types of exemptions from immunization requirements: Medical and Religious. Please see the Office for requirements.

The school will make every effort possible to protect the health and safety of the children. Cooperation of parents in helping to prevent the spread of communicable diseases is of great importance.
